Triiodothyronine therapy in the cardiac transplant recipient

  • In 52 patients undergoing open heart surgery at our center and at Groote Schuur Hospital, Cape Town, South Africa, including 22 undergoing cardiac transplantation, a significant fall in plasma free triiodothyronine (T3) has been observed during cardiopulmonary bypass (CPB). Triiodothyronine fell by a mean of 81% during the first 15 minutes after the initiation of bypass, remaining low throughout the procedure. The mean plasma level at the end of CPB was 0.81 (0.30) pmol/L, which was significantly lower than the pre-CPB level (P < .0001). After discontinuation of bypass, there was a slow recovery of plasma free T3, though more than 48 hours were required for normalization of the value.
