Tibiotalocalcaneal arthrodesis is a salvage procedure for end state pathologies of the hindfoot. Cases of primary failure can be a management challenge especially in failed intramedullary nail fixation. Inlay fibula strut grafting can be useful in such settings; however, many surgeons consider it a challenging approach. We describe our simplified approach for achieving successful fibula inlay strut grafting for hindfoot fusion. Using this approach, we achieved radiographic union in 81% (13/16) of hindfoot fusion revisions.
