Preanesthetic Management of the Hypertensive Patient

  • To the Editor: In a recent letter to the Editor1 Dr. W. T. Edwards commented on our recommendations regarding the discontinuation of antihypertensive medications in the preanesthetic period.2 I do not think that he represented our recommendations fairly. We noted that it is no longer necessary to discontinue antihypertensive medications routinely but added that “In the patient with uncomplicated mild to moderate hypertension, drug therapy need not necessarily be maintained during the pre- and post-operative periods…if circumstances permit.” We further stated that “medications be continued in the hypertensive patient with coronary disease or cerebrovascular disease or a history of hypertensive.
